Instructors: Please forward the completed Recommendation for Promotion Form (link to form is at bottom of this page) for your students that you desire to promote to Black Belts along with the $10.00 testing fee to Page Baptist, Chairman of the Board as soon as possible, but no later than 3 weeks prior to the date of the test. |
